It's being called one of Michigan's best kept secrets.
It's a free program that helps the elderly and disabled to transition into community-living.
Eva Balmer, 78, used to live in a nursing home, but in December through the help of a free state program she now moved to her own cass city apartment.
"I do like living here," says Balmer.
Staff is on hand to help her with basic home-making like washing clothes and running errands.
"They help me whether it's cleaning my house or cooking my meals," says Balmer.
It's called the MI Choice Waiver Program.
It transitions people from nursing homes to community-living, and it's catching on.
David Benjamin, program director for A+D Home Health Care says, "In 2005, there were only 37 transitions statewide and the program has grown to this year. We're going to do over 1,500 transitions statewide."
A+D Home Health Care in Saginaw will do around 200 of those.
Those interested can call A+D who will set up a consultation with a nurse and social worker.
They'll evaluate what the client needs.
A+D registered nurse Jeri Darby says it includes, "Anything from snow removal, whatever it takes to maintain someone in a community. We're able to provide that."
The help is not just for the client themselves but also relatives making difficult decisions.
A+D case manager Laura Izzo says, "Usually when I meet the family, they're stressed because they've been through so much with putting the family member in a nursing home, and that's the beautiful part of the program. I can provide support to the family members."
A+D says it's not technically assisted living, but still independent living that gives help where it's needed."
Officials say the program gives people their independence and actually saves the state money.
"On an average, nursing home care costs $180 a day. In community-based care, it costs an average of $60 a day," says Benjamin.
For Eva, it's a transition she's fitting well in to.
Cass City Senior Apartment manager Judy Hulburt says, "Eva takes care of herself. She joins in all the activities. She's a wonderful personality. She adds a lot to the place with her sense of humor."
Anyone who meets Michigan's nursing home eligibility criteria is qualified for the program.
